There could be many answers depending upon your likes and dislikes and as well as those of the person responding.  But here are my thoughts:

1. Spend a full day in the San Jose area and visit Poas volcano and the La Paz Waterfalls; or visit some of the National Museums if that interests you.

2. Drive to La Fortuna and stay for 3 nights. Stay at a hotel that has views of Volcan Arenal.  Do the hanging bridges tour and visit La Fortuna Falls,  as well as relaxing at a hot springs facility.

3. Visit Rio Celeste waterfalls (north of La Fortuna) and stay at La Carolina Lodge.  Hike to the falls one day, and either spend another day at the lodge, including horseback riding or hiking, or go whitewater rafting.

4. Drive to the Pacific coast for some beach time.  I like the Guancaste beaches in northwestern CR.  Try Tamarindo or Samara if you want to be in a town with a variety of lodging options and restaurant options.  Stay a few days to relax.  Option: while staying at the beach, visit Rincon de La Vieja National Park and take the mud pots hike or do the all day adventure tour at Hotel Guachipelin (zip lining,  horseback riding, and river tubing).

Then head back to San Jose.  Allow a day of travel between destinations.  Don’t try to pack too many sites into one trip.

(In lieu of Guanacaste beaches, you could go to Manuel Antonio National Park/ Quepos.  Spend a day visiting the park and the beach in the park, and another day at a beach outside the park or doing a boat tour.)
